As of February 20, 2012, any customer or member registering with FKC under a business name will need to provide FKC with the Statement of Information and/or Articles of Corporation of that business, as well as, a signed W-9 Form (Request for Taxpayer Identification Number and Certification Form). The person that shows as the officer of the business is by default the representative of the account with FKC International, Inc.  This is to insure that FKC is providing accurate information to the IRS and any other pertinent government departments.
